It noted that it had already submitted a written request to the FAA for 11 Boeing production systems in January 2023 for a special technical audit. The FAA rejected it in April. Former FAA chief Billy Nolen said at the time that the audit wasn’t necessary because the agency had implemented tools to replace those controls, the Seattle Times reported.
After the plate fell out, emergency inspections were ordered and it was found that several machines from the factory had the screws on the plates not tightened.

The FAA has already come under fire after two Boeing 737 MAX crashes in 2018 and 2019 that killed 346 people. That’s when it became clear that the FAA was letting the company do some of the inspections itself and was no longer overseeing them. At the same time, despite the reservations, the company did not make any changes to the flight leveling system and its decommissioning.
