Recovery from stroke is a long road, and although some patients have undergone medical procedures for a period of time, they often still have little understanding of the basic principles and concepts of treatment.

First of all, it is important to understand that the key to post-stroke rehabilitation lies in the recovery of brain nerves. Mobility problems after a stroke are not due to loss of muscle strength, but to damage to neurons in the brain, which prevents the smooth transmission of signals to control muscle activity. If mobility problems do not stem from muscle strength issues, why should patients be encouraged to undergo physical therapy to strengthen their muscles? In fact, the communication between brain and muscles is a bidirectional feedback: by stimulating the muscles, signals from bottom to top are returned to the brain. Repeated actions push the brain to generate new neural networks, thus achieving the goal of treating brain trauma. Traditional rehabilitation treatments are relatively devoid of a top-down, brain-muscle treatment model.

With the development of medical technology, there is a deeper understanding of the changes that occur in the brain after stroke, and treatment strategies have become more comprehensive. Simply put, the function of the damaged side of the brain is reduced and the healthy brain on the other side becomes stronger. Since the left and right brain have a control and balance mechanism, the goal of rehabilitation is to regulate the balance of activities in both brains. On the one hand it stimulates the brain cells in the damaged half area to promote the regeneration of the neural network; on the other hand it reduces the activity of the healthy half area to prevent it from inhibiting the recovery of the damaged half area. TMS therapy combined with a navigation system can stimulate normal and damaged brain cells to achieve the effect of regulating brain activity.

The ideal rehabilitation treatment for stroke should be a dual approach, with top-down and bottom-up feedback modalities playing complementary roles. My experience is that fighting spirit, perseverance, family support and encouragement, and taking the opportunity to coordinate with appropriate treatment plans are the keys to stroke recovery.
